The Substance Abuse and Mental Health Services Administration (SAMHSA) has released updated Federal Guidelines for Opioid Treatment Programs, aligning with the revised 42 CFR Part 8 regulations, which were effective April 2, 2024, with compliance required by October 2, 2024. The latest SAMHSA opioid guidelines aim to improve access to evidence-based treatments for opioid use disorder (OUD), emphasize practitioner discretion, and foster patient-centered care.
Key updates include:
- Admission Criteria and Telehealth: Simplified admission processes and expanded telehealth use, as outlined in the SAMHSA opioid guidelines.
- Clinical Decision-Making: Encouraging individualized, evidence-based care with practitioner discretion.
- Medication Handling: Procedures for methadone administration, dispensing, and take-home dosing.
- Trauma-Informed Care: Emphasis on integrating trauma-informed principles.
- Support for At-Risk Populations: Providing tailored care for vulnerable groups.
- Comprehensive Services: Ensuring medical, counseling, and vocational support are available.
- Continuity of Care: Strategies to ensure seamless treatment transitions, as recommended by the SAMHSA opioid guidelines.
In addition to these updates, the guidelines also aim to reduce stigma and barriers, fostering broader adoption of patient-centered approaches across all settings providing medication for opioid use disorder (MOUD).
Furthermore, these updates replace the 2015 guidelines and mark a shift toward modernizing care while improving patient engagement and outcomes.

To complement these guidelines, SAMHSA has also released a series of 14 brief videos designed to explain the key aspects of implementing the revised 42 CFR Part 8 regulations for Opioid Treatment Programs (OTPs). Notably, these videos offer a concise, high-level overview of the first major updates to these regulations in over 20 years, further reflecting the latest SAMHSA opioid guidelines.
The series aims to:
