Common Mistakes in a Quality Improvement/ Quality Assurance (QI/QA) Plan:

If you are an FQHC, look-alike, or thinking about becoming one, be sure to avoid these mistakes to be regulatory ready!

 

1. Jargon – Organizational jargon will not always align with Health Resources and Services Administration’s (HRSA) terminology. Try filling in your plan with verbiage from HRSA’s compliance manual. Make the reviewers’ job easy!

 

2. Quality Committee Structure – Make sure the Board of Directors (BODs) are overseeing the program. In hierarchical fashion, dictate responsibilities for each person/committee involved in quality. Make sure to include staff and site locations, they deliver the care!

 

3. Multiple Quality Plans – Some organizations divide the responsibility of different quality sectors among multiple departments. Try to combine these responsibilities into one QI plan by providing a summary of each sector in a single document. This will alleviate confusion and show synergy to your HRSA reviewer.
