HRSA has released the official Instruction Manual for the 2025 Uniform Data System (UDS) reporting period. Because the release occurred later than usual, Federally Qualified Health Centers (FQHCs) and Look-Alikes (LALs) should act quickly to review the updates. Early review ensures staff collect the correct data and Electronic Medical Record (EMR) systems are properly configured.

Key 2025 UDS Changes

Demographic Data (Table 3):
HRSA has removed the requirement to report Sexual Orientation and Gender Identity (SOGI) data.

Selected Diagnoses and Services Rendered (Table 6A):
New public health measures have been added to track:

  • Patients and visits involving tobacco cessation pharmacotherapies
  • Patients and visits involving medications for opioid use disorder (MOUD)
  • Patients and visits involving Alzheimer’s Disease and Related Dementia (ADRD) screening

Quality of Care Indicators (Table 6B):
A new measure evaluates whether patients with a newly diagnosed substance use disorder (SUD) received treatment within:

  • 14 days
  • 34 days

Alignment with CMS Measures (Tables 6B and 7):
HRSA has refined coding and clarified measurement criteria to better align with current CMS electronic clinical quality measures (eCQMs).

Next, collaborate with your EMR vendor or IT team to ensure systems are adjusted to meet updated specifications.
Then prepare training materials or hold briefings to update clinical and quality staff.
